National Home Health and Hospice organization is searching for a RN Case manager for the Rockford area to join their team. This is a full-time position.
The RN Case Manager is responsible for primary case management, including direct patient care, of assigned patients in the home setting. Documents care given and communicates with other heal! th-care team members. Coordinates and implements patient care specific to the age of the patient. Ensures quality of care is provided in efficient and safe manner. Accepts responsibility of patient care from patient's admission to discharge.
BSN preferred and electronic charting experience required.
Requirements
Minimum of one-year experience as a nurse in a home health or hospice setting, and current Illinois Nursing License required. .
