TitleHome Care Hospice Registered Nurse - DickinsonND LocationND-DickinsonJob Title Position Registered NurseRNReports To Director Home Health HospiceJOB DESCRIPTION SUMMARYThe registered nurse plansorganizes and directs home care services and is experienced in nursingwith emphasis on community health education experienceThe professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities ESSENTIAL JOB FUNCTIONS RESPONSIBILITIESPatient Care1Completes an initial assessment of patient and family to determine home care needsProvides a complete physical assessment and history of current and previous illness2Regularly re-evaluates patient nursing needs3Initiates the plan of care and makes necessary revisions as patient status and needs change4Uses health assessment data to determine nursing diagnosis5Develops a care planwhich establishes goals based on nursing diagnosis and incorporates therapeuticpreventiveand rehabilitative nursing actionsIncludes the patient and the family in the planning process6Initiates appropriate preventive and rehabilitative nursing proceduresAdministers medications and treatments as prescribed by the physician7Counsels the patient and family in meeting nursing and related needs8Provides health care instructions to the patient as appropriate per assessment and plan of care9Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient10Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload Communication1Prepares clinical notes and updates the primary physician when necessary and at least every sixty days2Communicates with the physician regarding the patient s needs and reports any changes in the patient s conditionobtains receives physician s orders as required3Communicates with community health related persons to coordinate the care plan Additional Duties1Participates in on-call duties as defined by the on-call policy2Ensures that arrangements for equipment and other necessary items and services are available3Instructssupervises and evaluates home health aide care provided Qualifications 1Registered nurse with current licensure to practice professional nursing in the state2Graduate of National League for Nursing accredited school of nursing3Maintains a current CPR certification4Current valid driver s license5Minimum of two years experienceat least one of which is in the area of public health or home care nursing is preferred6Self directed and able to work with minimal supervision7Demonstrates excellent observationproblem solvingverbal and written communication skillsnursing skills per competency checklist8Shows ability to organize and prioritize workload independently9Management experience not requiredResponsible for supervising home health aides10Prolonged or considerable walking or standingAble to liftpositionor transfer patientsAble to lift supplies and equipmentConsiderable reachingstoopingbendingkneelingor crouchingVisual acuity and hearing to perform required nursing skills Catholic Health Initiatives and its organizations are Equal Opportunity Employers CB JobNursing - RNStatusFull Time unknown
