The RN Case Manager provides management of care delivery to patients and families of all ages. This includes assessing, planning, and coordination of services to meet an individual’s health needs in a cost-effective manner. It also includes discharge planning, and reviewing the medical necessity and utilization of services that are required.
Education and/or experience equal to a bachelor’s degree in nursing. Current licensure as a RN in the state of ND required. Experience in the acute care/community health/nursing home areas typically acquired in four years preferred. Basic understanding of reimbursement issues and Diagnostic Related Groups. Knowledge of community services and other providers in the community is required. The inc! umbent must have the ability to access, organize and relay inf! ormation in meaningful ways using e-mail, word processing, spreadsheets and presentation software, and database management. Interqual criteria of medical necessity and intensity of service will be used to evaluate each admission. BLS
