Prairie St. John's,
a 91-bed psychiatric care facility located in Fargo, North Dakota,
has been providing services for children, adolescents and adults to
address mental health issues, chemical dependency or addiction and
co-occurring disorders since 1997. Prairie offers a full-service
psychiatric and addiction Continuum of Care. This Continuum of Care
includes inpatient hospitalization, partial hospitalization,
residential treatment, intensive outpatient services and clinic
Planner position available. The candidates selected will be
responsible for the following:
1. Coordinates the
patient's transition to life after the hospital stay.
2. Works
with residential facilities, treatment facilities, state hospitals
and agencies, nursing homes, homeless shelters and the
patient’s support systems to seamlessly facilitate a patient's
move to the next level of care.
3. Communicates with the
treatment team regarding the patient’s discharge plan
Requirements
The following are the job requirements for the
Discharge Planner position:
1. Bachelors Degree in Social Work
2. Licensed Social Worker in the State of North Dakota
3.
Highly organized
4. Capable of multitasking in a high-stress
environment
5. Has the ability to handle difficult situations
in a way that promotes the best outcomes for the patient
