Requires licensure as a Licensed Social Worker (LSW) by the North Dakota Board of Social Work Examiners (NDCC 43-41); and one year of professional human services work experience as a social worker, human relations couns! elor, developmental disabilities case manager, mental illness case manager, vocational rehabilitation counselor, activity therapist, addiction counselor, registered nurse, employment counselor, or a similar professional level position in the public or private sector; OR a masterâs degree in social work and licensure as a Licensed Social Worker (LSW) by the ND Board of Social Work Examiners.
Ability to visit persons in their homes using own transportation is required for this position. Flexible work hours and availability beyond traditional work hours (on occasion) are required.
Preference will be given to applicants who have had experience working with the elderly and/or disabled adults.

Summary of Work:
Essential functions of this position are: Case management of home and community based services for elderly and disabled, including: financial and functional assessment to determine eligibility, development of care plan, paperwork to authorize services, assisting client with other community reso! urces, regular monitoring to ensure adequacy of services, coordination ! with care providers and other involved agencies, and completion of service related administrative duties.
