Registered Nurse- Patient Care Coordinator The RN Patient Care Coordinator is a registered nurse who is responsible for coordinating, implementing, and evaluating the plan of care for each patient in their panel. This includes providing direct patient care such as on-call after-hours visits, admissions, routine home visits, death visits, continuous care visits, inpatient visits and discharges. The RN PCC also provides education and supervision. This position reports to the Director of Patient Services.
Clinical Duties:
To know the condition of the patient and family needs, what medications the patient is on, and what durable medical equipment is being used
To obtain orders for new medications and treatments
To educate patient and family related to disease progression, patient prognosis and dying process
To evaluate changes in patient condition and family needs and/or results of new medications and treatments
Administrative Duties:
Provide accurate and timely documentation of all nursing interventions and visits a with patients and their families or other caregivers
Documentation of education provided and plans of care
Update the Plan of Care in a timely manner to ensure optimum patient and family care and attend IDG
Inform all team members of the content of the patient-driven Plan of Care and ongoing changes to that Plan of Care
Keep the ! attending/admitting physician informed about patients condition
Assist with any Quality Assurance projects
Supervisory Duties:
Provide written instructions for home health aides and assessing the services provided by home health aides in the patients home
Registered Nurse- Patient Care Coordinator
A registered nurse with a current license to practice as a registered professional nurse in the state of Illinois
Ability to lift 50 lbs or greater
Current in BLS
Demonstrated knowledge of health care policies and program responsibilities
Preferred experience in hospice or home health
