The medical home health coach (MHHC) supports the patient-centered medical home (PCMH) by working with patients, families, providers, other providers, and staff to promote timely access to needed care, providing daily continuity of care coordination, and coaching patients, families, and caregivers to understand the patientÂ's care plan and self-care management responsibilities.
Education:
BachelorÂ's level preparation as a nurse or the equivalent with appropriate past experience in healthcare.
Licensure:
Current, active, and unrestricted registered nursing license or certification in a health or human services discipline that allows the person to conduct independent assessments as permitted within the scope of practice of the discipline required.
Experience:
Three to five yearsÂ' experience in a primary care practice with duties in leading care coordination, or the equivalent.
Interpersonal Skills:
Excellent communication skills. Willing to work as part of a team. Must be committed to compassionate and high quality care.
Physical Effort:
Medium: Involves a maximum physical effort requirement of 50 pounds.
Hours of Work:
30-40 hours per week. No weekends.
Authorized to work in the U.S. is a precondition of employment.
