Summary of Work



One position will provide case management services to individuals in our Treatment Mall with mental illness and or co-occurring disorders of mental illness and chemical dependency the other will serve our Medical Services unit. Basic case management duties include but are not limited to assisting individuals with the following: securing basic needs, obtaining and maintaining entitlements, coordination of services and or skill building for activities of daily living, access and navigation of services and resources in the community, facilitation of group work and or supportive counseling to promote and elicit change for recovery and managem ent of mental illness and or co-occurring disorders.

Case management will involve considerable collaboration with internal and community providers ranging from psychiatrists, nurses, therapists, addiction counselors to families, landlords, probation officers, housing and employment specialists.

The successful candidate will participate in practicing case management within a stage based model of treatment and be able to develop competency in using stage based interventions, Motivational Interviewing, person centered treatment planning and intensive community based case management practices.




Minimum Qualifications



Requires a bachelor’s degree in social work, psychology, counseling (including addiction counseling & LMFT), nursing, occupational therapy, vocational rehabilitation, therapeutic recreation, or human resource management (human services tra ck), and two years of experience working with special population groups in a direct care setting; OR a master’s degree in one of the fields listed. Prefer master’s degree.

Requires a valid driver’s license, the ability to drive and a reliable form of transportation. Must be able to work 8 am-5 pm Monday through Friday.
