The registered nurse is responsible to implemen! t the nursing process within Sanford Health and the continuum of care. The nursing process is utilized to assess, diagnose, establish a plan with outcome criteria, intervene, evaluate, and document human responses to actual or potential health problems. This practice includes dependent, interdependent and independent functions. Reports to As Assigned Education, Experience and Skills Requirements Must have graduated from an accredited school or college of nursing. Bachelor's degree in nursing is preferred. Position requires one year of experience, preferably in home health, as well as excellent communication, problem solving and critical thinking skills. Must be able to work independently. Licensure/Certification Requirement Requires current licensure as a Registered Nurse in the state, or states, of practice. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Job Specific Competencies
Develops a ! written plan of care in accordance with the patients diagnosis! and family needs and physician orders. Coordinates the development of this plan with other disciplines providing care for patient. Modifies the written plan of care as necessitated by changes in patients condition.
Performs case management functions when assigned as primary clinician.
Documents observations, assessments, interventions of visits, phone calls and conferences in the patients clinical record on a timely basis.
Observes and follows the duly adopted standards, policies, directives, and orders of the Board of Nursing as they may relate to the registered nurse.
Administers nursing care in accordance with the established plan of care including therapeutic treatment, teaching, counseling and supervising patient and/or care givers in peforming specific procedures. Uses appropriate disciplines and community resources for optimum patient care.
Utilizes critical thinking skills when assessing the physical, psychosocial, spiritual and environmental ne! eds of assigned patients.
Discharges the patient when appropriate, notifying the physician. Instructs the patient and/or family how to access necessary follow-up.
Keeps updated on Medicare/Medicaid regulations and other regulatory agencies (ex. state, JCAHO).
Participates in patient care conferences with other members of the interdisciplinary team.

Evaluation: Evaluates the responses to nursing interventions by documenting and communicating evaluation data to appropriate members of the health care team, utilizing data as a ! basis for reassessing client health status, measuring outcomes and goal attainment, modifying nursing diagnoses, revising strategies of care and prescribing changes in nursing interventions.
Documentation: Thoroughly documents patient information in an accurate and timely manner, according to organizational policies, procedures and guidelines.
Patient Education: Assesses patient needs and designs, implements and evaluates a teaching plan specific to individual needs or patient groups. Age Related Competencies

This position is classified as a Category I position under OSHA guidelines with high risk of exposure to bloodborne pathogens and other potentially infectious materials.
