Duties:
• Serves as an advocate in the care coordination process by
supporting decisions, cla! rifying options, and problem solving with the
interdisciplinary team.
• Serves as the Mental Health Treatment Coordinator in
facilitating resolutions of identified needs.
Maintains regular contact with veteran as indicated. Understands treatment goals, ensures
coordination and development of treatment planning includes veteran input,
monitors and facilitates timely treatment planning updates.
• Provides additional support between clinic visits with
their provider (e.g. medication follow-up phone calls to assess compliance,
response, and reinforce patient/family teaching).

• Utilizes triage techniques and case management theories to
enhance patient and provider satisfaction.

• Collaborates with patient and the health care team to
evaluate and update changes in the plan of care and management of the patient's
conditions. Recognizes complex
situations that impact patient care and intervenes by protocol-b! ased interventions,
communication with the patient's healt! h care team or care within the community
or Emergency Room.
