Case Manager Specialist

For complete informations about this job opportunity please give attention to these descriptions. Case Manager provides case management services to adults/ youth and their families. It is designed to serve clients that are involved with the assigned program and use Psychiatric services who have shown a consistently stable state with the use of psychopharmacology and have completed therapy/counseling and/or need minimum contact from caseworker. The Case Manager will provide services deemed medically necessary, case management, treatment planning, assessment etc.
Essential Responsibilities:
- Work hours prescribed and any additional hours d! eemed necessary for the responsibilities assigned.
- De! velop and complete required assessments within time frame, develop and implement interim and long term treatment plan with client, coordinate psychiatric services and treatment along with psychiatrist and nurse
- Coordinate and collaborate with all assigned team members, psychiatrist and nurse regarding comprehensive services provided to participants, including complete information sharing and coordination of delivery of services
- Provide referral and linage to additional internal and/or external services based on need or presenting severity of symptoms
- Graduate clients from services whenever possible by lining them to their primary care providers for continued prescribing needs
- Consult with LPHAâs, psychiatrists, psychologists and nurses for disposition
- Have full knowledge of various programs at Rosecrance and the referral process
- Provide case management services and all other medically necessary services according to Ru! le 132 and Rosecrance policies and procedures
- Coordinate immediate treatment and complete appropriate referrals for intervention when needed
- Meet the direct hour standards of RBC
- Complete documentation in a timely manner
- Attend required meetings and training
- Other duties as assigned
- Bachelorâs Degree in counseling, psychology, social work or another related filed required, Masterâs preferred. Knowledge and experience with psychopharmacology preferred.
- The ability to communicate clearly verbally and write concisely and legibly
- Ability to make recommendations and linkages based on reported side effects, symptoms and/or concerns.
- Ability to assess mental status based on reported and observed symptoms
- Ability to quickly establish rapport with others
- Ability to perform in an ethical manner
- Ability to follow written and oral instruction
- Valid driverâs license!
